Requirements to Complete Wind Mitigation Survey Form for Your Insurance Provider
A form will be provided by your specific insurance agency to comply with the state's requirements. If not, the general state form will be used and accepted by your insurer. According to Rule 94 your insurance company is required to honor this program.
